Open a picture - duplicate - black and white - pencil sketch OR ink sketch (whatever you prefer) - remove background - add new layer under the sketch - make it black or grey or so - Add a new layer between the black/grey background and the sketch - Paint the hand in middle tones (tones between the highlights and lowlights) - delete OR make the original invisible - add a new layer - use the line tool to create small lines around the hand
The sketch and the lines are only a GUIDE, to see if you are on track

Add a new layer called shadows on top of the basic middle tones hand - add a new layer on top of shadows - paint a black line where shadow is needed - Gaussian blur - remove black shadow from places it doesn't need to be ( as seen with the highlights at step 15/16 - merge layer down to shadows
Every time you want to add a shadow or a highlight, do it on a new layer and merge it down when you are satisfied about that highlight or shadow!
